Job Description

Role:

Nurse Practitioner with experience in Behavioral Medicine to join a Family Practice. We are well-established, with a focus on providing integrated health and wellbeing services in a pleasant environment.

The clinic offers a broad range of services including men’s and women’s health, interventional pain procedures, pediatrics and general medical treatment.

General responsibilities:

  • Responsible for providing outpatient care to patients showing a change or decline in mental status, chronic or urgent medical conditions;
  • Work independently and with other members of the medical team;
  • Take part in an interdisciplinary team that provides primary care and auxiliary services;
  • Perform patient’s age-appropriate histories and physicals, risk factor assessment, ordering of therapeutic and diagnostic tests; and attend to consultations and appointments;
  • Prescribe pharmacologic and non-pharmacologic treatment modalities following the clinic’s policy, formulary and relevant regulations of the State;
  • Consult with physicians on cases that are beyond the scope;
  • Put all relevant patient data in medical records and case presentations;
  • Formulate, execute and review healthcare education delivered to patients and families for diseases management and/or prevention;
  • Ensure smooth communication with other health professionals for effective delivering of clinic service to patients;
  • Practice in accordance with State and Federal rules and regulations and the nursing standards of care (assessment of health status, diagnosis, development of plan of care and treatment, implementation of treatment plan, and evaluation of patient status); and
  • Ensuring the clinic maintains its profile as a community leader in health care.

Minimum qualifications:

 

  • Possess and maintain graduate professional nurse practitioner registration in a state, which must be unrestricted, current, complete, and active;
  • Possess clinical experience with Behavioral Medicine;
  • Be compassionate, sensitive, and possess strong reasoning and thinking skills; and
  • General computer skills; able to manage EHRs, timely keep patients’ records and willing to learn a new HER program if needed.
