We are currently seeking a Patient Assistant to work at our 400 Broadway location.
Position Summary: The Patient Assistant will be responsible for direct services to clients living with HIV/AIDS as a part of the care coordination team; This will include field base outreach, appointment reminder calls, and escorting clients to appointment when needed; This individual will also identify and recruit people who are living with HIV/AIDS to facilitate early access to treatment and social services.
Qualifications: The Patient Assistant must have a high school diploma/GED with some working experience. Some college education with one (1) year of working experience is preferred; This individual should have the ability to follow instructions, meet deadlines, communicate well with others, handle multiple tasks, etc.
